We have set rates for BLS and ALS and then we bill for mileage. You can always contact billing and dispute your charges in writing.

You should have all the information on a bill that states where to submit payment
Q: How do I know if insurance paid on my bill?
Your bill will have the amount, if any, paid by insurance and then will have what portion is yours to pay. It will also show if insurance denied payment. If your claim is denied, then you will know that any remaining balance is your responsibility.
