DO NOT use this form if you are requesting an on-street neighborhood parking permit in the campus area. This form should be filled out only by the HOMEOWNER/LANDLORD

Criteria for neighborhood on-street parking permits:
- We must have a physical copy of your lease with your name and address. It also must state the beginning and end date of your lease
- Permit expires JULY 31 of each year.
- If your license plate changes or you have a short term rental car you must notify parking enforcement immediately.
- Only one permit per residential address. If your house was a single family that has been converted into a multiple units. There will only be one permit issued on first come first serve basis.
- If you reside in the Village Promenade, there are no permits issued as they have a parking structure and do not meet the above criteria.
- Once your application has been approved for a street parking permit, and your street has two hour parking on one side with Restrictions on the other side, you are exempt from the two hour parking limit.
- Your street parking permit is only valid for the color restriction for where you reside. (For example, a red permit does not allow you to park in a restricted blue area)
